This service includes evaluation, vocational training, professional counseling, work opportunities, habilitation training, protective and socio-legal services
Professional and paraprofessional counselors are available to provide group and individual counseling and crisis case management for adults and children. Medical professionals are available for medication management. School and clinical based services are also available
NISH/AbilityOne contracts are managed by Pathfinder to provide competitive jobs for individuals with disabilities.
Pathfinder also is a vendor to provide Supported Employment to help individuals with intellectual disabilities procure competitive employment within integrated settings
Home & Community Based Services
This is a home-based program which provides individual services within the community for persons who might otherwise have to seek services in an institution
An Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID) is an Active Treatment and Health care related program designed to assist individuals to maximize their independent living potential. All ICF/IID programs are staffed 24 hours a day, 7 days per week.
Supervised living, Apartment living and Rental Assitance Vouchers are some of the services Pathfinder provides through our Residential Services Office
Supported Employment is any service provided to consumers with significant disabilities, to assist them in obtaining and maintaining employment at an integrated labor site
provided to assist enrollees in Pathfinder programs. Transportation is also available to assist enrollees in maintaining employment outside of Pathfinder
